Updating of Customer Information


In compliance with BSP Circular No. 618 on The Basic Standards in the Administration of Trust, Other Fiduciary and Investment Management Accounts requiring the review and updating of account documents, the Anti-Money Laundering Law (R.A. No. 9160, as amended) on client Identification, and the U.S. Foreign Account Tax Compliance Act (FATCA) on the declaration by US persons, BPI Asset Management and Trust Corporation has the fiduciary duty to ensure that all the documents of our clients are updated for the prudent management and monitoring of accounts.
 
In line with this, we request you to update your customer records with us.
 
For UITF accounts, kindly coordinate with your branch of account for the updating of the following:
 
  1. Customer Information Sheet
  2. FATCA Information Sheet
  3. Client Suitability Assessment
  4. Suitable Products List
  5. Risk Disclosure Statement
  6. Omnibus Account Opening Form (Includes the Participating Trust Agreement)
  7. Submission of a copy of a valid government-issued ID with signature.
 
For IMA/Trusteed/Other Fiduciary accounts, you may get in touch with our Accounts Review and Documentations Officer, Martin Jerald P. Juco (with contact details +632 816-9823 or +63 927 1359702 and/or wmcs_docs@bpi.com.ph) for the updating of the following:
 
  1. Customer Information Sheet
  2. Specimen Signature Card
  3. Client Suitability Assessment (CSA)
  4. Investment Policy Statement (IPS)
  5. Risk Disclosure Statement (RDS)
  6. Trust/IMA Agreement
  7. Account Opening and Instruction Sheet
  8. Valid ID of Trustor/Principal/Beneficiary/ies
  9. US Person Declaration for Individual Accounts
